What is an AI medical scribe and how does it work?

An AI medical scribe (also called an ambient scribe) is a tool that turns a recording of a consultation, or a doctor's dictation, into a transcript and a draft clinical note. The doctor reviews and corrects the draft before it goes into the medical record.

The goal is simple: less time typing, more time with the patient.

How an AI scribe works

Almost every AI scribe works in three steps:

  1. Speech recognition. A speech recognition model turns the recording into text. Medical speech is demanding: grammar, accents, Latin terms and drug names in the same sentence.
  2. Drafting the note. A language model picks out what is clinically relevant (complaints, history, examination, diagnosis, treatment) and arranges it into the structure of the note.
  3. The doctor's review. The doctor reads the draft, fixes mistakes and approves the final text.

With an ambient recording (a conversation with the patient), the tool has to tell what the doctor says from what the patient says and leave out what is not clinically relevant. With a dictation, the doctor speaks the finding, so the task is closer to an accurate transcript.

Where it helps

  • Documentation after the visit. Instead of writing the note from memory at the end of the day, you have a draft right after the conversation.
  • Consistent structure. A template makes sure every note has the same sections, which helps teams that work in shifts.
  • Dictating reports. Longer reports and discharge letters are faster to say than to type.

What to watch for

  • Recognition errors. Always check numbers, doses and drug names. A misheard dose is a clinical risk.
  • Invented content. Language models sometimes add something that was never said. The note must contain only what happened in the visit.
  • Data protection. A recorded consultation contains health data, a special category of personal data under the GDPR. You need to know where it is processed, how long it is kept and who can access it. See our GDPR checklist for AI scribes.
  • Responsibility. An AI scribe does not make clinical decisions and does not replace the doctor. The doctor who approves the note is responsible for its content.

Why language matters

Many AI scribes were built for English first and added other languages later. You can see it in how they handle medical terms, in the grammar of the note, and in whether they write the note directly in your language or translate it from English. Before choosing a tool, try it on a real dictation from your specialty, with numbers and drug names.

MedGlas

MedGlas is an AI medical scribe that works in Croatian, English, German, French, Italian, Spanish, Polish, Dutch, Portuguese and Slovenian. You record in the browser on your phone or computer, and when you stop you get a transcript and, in conversation mode, a draft note in your own template, written in your language.
